Healthier You Diabetes Prevention Programme

What is the NHS Diabetes Prevention Programme? 

The NHS Diabetes Prevention Programme is a national scheme set up by NHS England. It is a behaviour change programme aimed at patients who have been identified as at risk of developing diabetes (patients with non-diabetic hyperglycaemia). This tailored healthy nutrition and exercise support programme aims to delay or prevent the onset of type 2 diabetes. It is provided locally by Independent Clinical Services (ICS) Health and Wellbeing. 

How can I refer patients to the NHS Diabetes Prevention Programme? 

1.         Sending invitation letters (one off task)

The practice first needs to sign the Data Processing Agreement. The practice then runs the retrospective search using the guidance produced by the CCG Primary Care Information Team to identify patients who are eligible to be sent an invitation letter. Practices can email the team CAPCCG.primarycareinformation@nhs.net for copies of the guidance.

Following feedback from practices we have reduced the search term to 6 months so there is less of a gap between a blood test being done, and the patient receiving the invitation letter through the post.

2.         Proactive referrals (ongoing)

Patients are offered referral to the programme by the GP or Practice Nurse as appropriate, e.g. in a consultation when a recent blood test identifies the patient as being eligible for referral to the NHS Diabetes Prevention Programme.  SystmOne and EMIS Web can be set up to prompt users when they access a record that a patient is eligible and to suggest offering referral to NHS Diabetes Prevention Programme.
The Primary Care Information Team will advise and support practices with this.

3.         Via NHS Health Checks

Patients are identified through the NHS Health Checks Programme and offered a referral by the team. The referral template to NHS Diabetes Prevention Programme is available within the clinical system.
